Matrix Home Care is seeking experienced and compassionate Speech Therapists / Speech-Language Pathologists (SLP) to provide home health therapy visits throughout West Palm Beach, Jupiter, Port St. Lucie, and surrounding areas.

Position Summary
The Speech Therapist / Speech-Language Pathologist will be responsible for evaluating patients, developing individualized treatment plans, providing skilled speech therapy services, educating patients and families, and coordinating care with the clinical team. The ideal candidate will have home health experience, strong documentation skills, and the ability to deliver excellent care in the home setting.
Responsibilities
Complete speech therapy evaluations and follow-up visits in the home setting
Develop and implement individualized therapy plans of care
Provide skilled treatment for communication, speech, language, swallowing, and cognitive-linguistic needs
Educate patients and families on therapy goals, home programs, safety, swallowing precautions, and strategies to improve independence
Monitor patient progress and communicate changes to the clinical team
Coordinate care with physicians, nurses, therapists, caregivers, and the interdisciplinary team
Complete accurate and timely documentation
Follow agency policies, procedures, and regulatory requirements
Qualifications
Active Florida Speech-Language Pathologist (SLP) license
Previous home health experience preferred
Experience with dysphagia, swallowing therapy, cognitive-linguistic therapy, and communication disorders preferred
Strong clinical assessment, communication, and documentation skills
Ability to work independently and manage assigned visits
Reliable transportation and willingness to travel within assigned service areas
Current CPR/BLS certification preferred
Must be able to complete required onboarding requirements
Background Screening Requirement
All applicants must successfully complete a Level 2 background screening in accordance with Florida Statute Chapter 435 and AHCA requirements prior to working with patients.
