Matrix Home Care is seeking experienced Registered Nurses (RN) for Start of Care and home health visits throughout West Palm Beach, Jupiter, Port St. Lucie, and surrounding areas. This is a great opportunity for nurses who are compassionate, dependable, and comfortable working independently in the home care setting. The Registered Nurse will provide skilled nursing care, complete assessments, perform Start of Care visits, and support patients in maintaining their health, safety, and independence at home.

Position Summary
The Registered Nurse will be responsible for providing skilled home health nursing services, including Start of Care assessments, patient evaluations, follow-up visits, care coordination, documentation, and communication with the care team. The ideal candidate will have home health experience, strong clinical judgment, and the ability to provide excellent patient care in the home setting.
Responsibilities
Complete Start of Care visits and skilled nursing assessments
Provide direct patient care in the home setting
Perform follow-up nursing visits as assigned
Monitor patient condition and report changes as needed
Educate patients and families on care plans, medications, safety, and disease management
Coordinate care with physicians, therapists, caregivers, and the clinical team
Complete accurate and timely documentation
Follow agency policies, procedures, and regulatory requirements
Qualifications
Active Florida Registered Nurse (RN) license
Previous home health experience preferred
Start of Care and OASIS experience preferred
Strong assessment, communication, and documentation skills
Ability to work independently and manage assigned visits
Reliable transportation and willingness to travel within assigned service areas
Current CPR/BLS certification preferred
Must be able to complete required onboarding requirements
Background Screening Requirement
All applicants must successfully complete a Level 2 background screening in accordance with Florida Statute Chapter 435 and AHCA requirements prior to working with patients.
