Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses (LPN) to provide skilled home health visits throughout West Palm Beach, Jupiter, Port St. Lucie, and surrounding areas.

The Licensed Practical Nurse will support patients with skilled nursing care, follow the established plan of care, and help patients remain safe, comfortable, and independent in their homes.

Position Summary
The Licensed Practical Nurse will provide skilled nursing services under the direction of the Registered Nurse and clinical team. Responsibilities may include patient care visits, medication management support, wound care, monitoring patient conditions, patient and family education, and timely documentation.
Responsibilities
Provide skilled nursing care to patients in the home setting
Follow the patient’s individualized plan of care
Monitor and report changes in patient condition to the Registered Nurse and clinical team
Assist with medication reminders, medication administration, and treatment as ordered within LPN scope of practice
Provide wound care and other skilled nursing services as assigned
Educate patients and families on care needs, safety, medications, and disease management
Communicate with physicians, nurses, caregivers, and the interdisciplinary team as needed
Complete accurate and timely documentation
Follow agency policies, procedures, and regulatory requirements
Qualifications
Active Florida Licensed Practical Nurse (LPN) license
Previous home health, private duty, skilled nursing, or facility experience preferred
Wound care experience preferred
Strong communication and documentation skills
Compassionate, dependable, and patient-focused
Ability to work independently in the home care setting
Reliable transportation and willingness to travel within assigned service areas
Current CPR/BLS certification preferred
Must be able to complete required onboarding requirements
Background Screening Requirement
All applicants must successfully complete a Level 2 background screening in accordance with Florida Statute Chapter 435 and AHCA requirements prior to working with patients.
